Find a Long-Term Care Plan

Here you will find a listing of long term care plans. 

First, select your borough or county and click on the 'go' button. Scroll down the page until you find the kind of long term care plan you want. The three types of plans are PACE plans, Medicaid Advantage Plus and MLTC Medicaid plans.  You may visit the plan's website or call the plan if you have any questions. You can also call New York Medicaid Choice. Not all New York State counties are listed at this time.
