More tips to choosing a Long Term Care Plan

All plans will arrange your home care and other long term care services but some plans work in different ways than others. Decide which one of the two statements below best describes what you want. I don’t want to change doctors or anything with my Medicare health services. Ask about: MLTC Medicaid Plans

 I don’t mind changing doctors if I have to.  Read about the other two kinds of Long Term Care Plans. Choose the one you like.  

1. I want a plan that has an adult day center that offers social activities and where a team of doctors, nurses, social workers and other providers will be in charge of my care. My team would also arrange for my home care services. Ask about: Program of All-Inclusive Care for the Elderly (PACE). You must be 55 or older. 

2. I want a plan for both my home care and Medicare health services. I want my Primary Care Provider to be in the plan.  Ask about Medicaid Advantage Plus.

 

What happens if you don’t choose a plan

You must choose a health plan if you received an enrollment packet from New York Medicaid Choice.  The letter in your packet tells you when you must join a plan. If you don’t choose a plan by the date listed in your letter, the Medicaid program will choose a plan for you.  It is important to choose!
