C-YES: Helping Children and Youth Access Home and Community Based Services

The Children and Youth Evaluation Service (C-YES) is a statewide program that determines whether a child or youth qualifies for home and community based services (HCBS) through the Children’s Waiver.

HCBS are Medicaid services provided in the home or in the community for children and youth who need extra care and who want to avoid going to the hospital or long term facility. More information can be found in the HCBS Brochure: omh.ny.gov/omhweb/bho/docs/hcbs_brochure_english.pdf

How does C-YES help?

New York State has designated C-YES to assist children and youth without Medicaid who are seeking home and community based services through the Children’s Waiver. Registered nurses meet with the child or youth to determine whether they meet the criteria for HCBS. Families can also receive assistance in applying for Medicaid.    

How to refer a child/youth without Medicaid to be evaluated for HCBS through the Children’s Waiver?

Individuals and families can self-refer by calling C-YES at 1-833-333-2937. They will be sent a Referral Form to fill out and return in a pre-paid envelope.
