New York Medicaid Choice is now in Jefferson, Lewis, St. Lawrence and Warren counties

New York, NY- Starting October 1, 2012, Medicaid clients residing in Jefferson, Lewis, St. Lawrence and Warren counties will begin receiving managed care enrollment letters from New York Medicaid Choice, the State’s managed care enrollment program. Most residents with Medicaid from these counties are now required to join a managed care plan for their health services.  Individuals will have 30 days within receipt of their enrollment letter to select a health plan, or be assigned to a plan.

New York Medicaid Choice’s enrollment packet includes a booklet with information about managed care and a list of health plans. The brochure is produced in English, Spanish, Russian and Chinese and is also available on CD.

County residents may visit their local Social Services Office to meet with a New York Medicaid Choice counselor or contact the Customer Service Center to enroll in a health plan over the phone by calling 1-800-505-5678.  TTY:  1-888-329-1541, Monday - Friday, from 8:30 am – 8:00 pm and Saturday, from 10:00 am to 6:00 pm.  All languages are spoken.
